Family-Centered Triadic Communication in Enhancing Diabetes Treatment Adherence

Abstract

Diabetes Mellitus is a chronic metabolic condition whose effective management hinges not only on medical treatment but also on sustained patient adherence. Yet, low compliance remains a persistent challenge in Indonesia. Moving beyond traditional biomedical and behavioral perspectives, this study explores the underexamined role of triadic communication among doctors, patients, and families in shaping adherence within primary care. Using a qualitative case study at Pekalangan Community Health Center in Cirebon, we conducted in-depth interviews and observed clinical interactions to analyze verbal, nonverbal, and mediated communication patterns. Findings reveal that collaborative, family-based communication (characterized by clear explanations of glucose targets, empathetic eye contact, supportive gestures, and the use of informational and motivational aids) significantly strengthens patient engagement. At the same time, barriers such as medical jargon, emotional distress, limited consultation time, and weak family involvement undermine therapeutic relationships.
